Whether smoking is bad or not is irrelevent, so many things are bad for health, obesity epidemic is because of life style choices, tom some doc gonna decide he/she not gonna give treatment cuz pts is obese? All doc can do is advise and cajole and no point he/she should be allowed to not tx any patient unless he/she has a solid medical ground to stand on.

No one stopping physician to speak out against smoking, obesity or whatever they can do to thier heart content but they can't and shouldn't be allow to refuse treatment even non-emergency treatment. They are neither the moral police nor they are arbitator of how society should live.
smoking being bad for your health is not a moral judgement, it is a fact. rest of your points i've already addressed.
